भिवाड़ी: भारत में ओरल कैंसर एक गंभीर स्वास्थ्य समस्या बन चुका है। यह पुरुषों में सबसे आम और महिलाओं में चौथा सबसे आम कैंसर है। चिंताजनक बात यह है कि लगभग दो-तिहाई मामलों में इसका निदान तब होता है जब कैंसर काफी बढ़ चुका होता है। इसलिए इसके शुरुआती लक्षणों की पहचान और समय रहते इलाज बेहद ज़रूरी हो जाता है। ओरल कैंसर के कारणों, लक्षणों और बचाव के तरीकों को जानना सभी के लिए आवश्यक है, ताकि समय रहते निदान और उपचार सुनिश्चित किया जा सके।


ओरल कैंसर उस स्थिति को कहा जाता है जिसमें मुंह के भीतर के किसी भी हिस्से में कैंसर विकसित होता है। इसमें गाल (बक्कल म्यूकोसा), मसूड़े, जीभ, मुंह का निचला भाग, ऊपरी व निचला जबड़ा, रेट्रोमोलर ट्राइगोन, हार्ड पैलेट और होंठ शामिल हैं। यह बीमारी भारत में पुरुषों में मृत्यु का सबसे आम कारण है। हर साल लगभग एक लाख नए मामले सामने आते हैं, जिनमें से लगभग 50 प्रतिशत मरीजों की मृत्यु हो जाती है, जो इसकी गंभीरता को दर्शाता है।


मैक्स सुपर स्पेशलिटी हॉस्पिटल, द्वारका के सर्जिकल ऑन्कोलॉजी (हेड एंड नेक) विभाग की एसोसिएट डायरेक्टर एवं यूनिट हेड डॉ. शिल्पी शर्मा ने बताया कि “ओरल कैंसर के प्रमुख कारणों में तंबाकू चबाना और धूम्रपान शामिल हैं। शराब का सेवन भी ओरल कैंसर का एक बड़ा कारक है। यदि तंबाकू और शराब का सेवन एक साथ किया जाए तो कैंसर होने का खतरा कई गुना बढ़ जाता है। हालांकि, यह जानना भी जरूरी है कि ओरल कैंसर उन लोगों को भी हो सकता है जो न तंबाकू का सेवन करते हैं और न ही शराब पीते हैं। ऐसे मामलों में तीखे दांतों द्वारा गाल या जीभ पर बार-बार लगने वाली चोटें भी कैंसर का कारण बन सकती हैं। इसके अलावा, खराब मौखिक स्वच्छता भी ओरल कैंसर से जुड़ी हुई है। ओरल कैंसर के सामान्य लक्षणों में मुंह के भीतर एक न भरने वाला घाव, बिना दर्द के कोई वृद्धि या खुरदरी सतह, आवाज में बदलाव, खाने में कठिनाई, जीभ की गति में रुकावट और अचानक दांतों का हिलना या गिरना शामिल हैं। गले में गांठें या सूजन भी ओरल कैंसर का संकेत हो सकती हैं।“


ओरल कैंसर का निदान एक साधारण बायोप्सी परीक्षण द्वारा किया जाता है। रोग के प्रसार का आकलन करने के लिए सीटी स्कैन या एमआरआई जैसे इमेजिंग टेस्ट भी आवश्यक होते हैं। इलाज की बात करें तो, ओरल कैंसर का प्राथमिक उपचार सर्जरी है। यदि प्रारंभिक अवस्था में कैंसर का पता चल जाए तो सर्जरी सरल होती है और रोगी जल्दी स्वस्थ हो सकता है। वहीं, उन्नत मामलों में सर्जरी के साथ-साथ रेडिएशन थेरेपी और कुछ मामलों में कीमोथेरेपी की भी आवश्यकता होती है।


डॉ. शिल्पी ने आगे बताया कि “ओरल कैंसर को रोका जा सकता है। चूंकि तंबाकू और शराब इसके मुख्य कारण हैं, इनका सेवन न करने से इस बीमारी से बचाव संभव है। इसके अतिरिक्त, जो लोग तंबाकू या शराब का सेवन करते हैं, उन्हें नियमित रूप से विशेषज्ञ डॉक्टर से अपने मुंह की जांच करानी चाहिए ताकि किसी भी दंत चोट या पूर्व-कैंसर अवस्था की पहचान समय पर हो सके। पूर्व-कैंसर अवस्थाओं में ल्यूकोप्लाकिया (सफेद धब्बा), एरिथ्रोप्लाकिया (लाल धब्बा), ओरल सबम्यूकस फाइब्रोसिस और इरोसिव लाइकेन प्लानस जैसी स्थितियां शामिल हैं, जो समय के साथ कैंसर में परिवर्तित हो सकती हैं।“


सकारात्मक बात यह है कि प्रारंभिक अवस्था में निदान होने वाले ओरल कैंसर का सफलतापूर्वक इलाज किया जा सकता है। हालांकि उन्नत अवस्था में भी इलाज संभव है, लेकिन सफलता की संभावना अपेक्षाकृत कम होती है। आज चिकित्सा क्षेत्र में हुए महत्वपूर्ण विकास ने कैंसर रोगियों के जीवन की गुणवत्ता को बेहतर बनाने में उल्लेखनीय प्रगति की है।


ओरल कैंसर को यदि समय रहते पहचाना और सही तरीके से इलाज किया जाए तो इसे पूरी तरह से ठीक किया जा सकता है। रोकथाम और उपचार को प्राथमिकता देकर हम इस जानलेवा बीमारी के प्रभाव को कम कर सकते हैं और अनगिनत लोगों की जान बचा सकते हैं।

Treating one of the rarest and complex conditions, doctors at Max Super Speciality Hospital, Saket have successfully treated a 16-year-old boy from Rohtak who was suffering from Dysphagia Lusoria — a rare condition caused by an abnormal artery pressing on his food pipe. The life-changing surgery was done using advanced robotic technology, ensuring minimal pain and quicker recovery for the young patient.


The surgical team was led by Dr. Rajneesh Malhotra Vice Chairman & Head – CTVS, Max Super Speciality Hospital, Saket, along with along with the team, who performed a robotic-assisted repair of the abnormal right subclavian artery — a key blood vessel responsible for supplying blood to the right arm.


Dr. Rajneesh Malhotra, Vice Chairman & Head – CTVS, Max Super Speciality Hospital, Saket said “The boy had been facing increasing difficulty in swallowing solid food and experienced a feeling of heaviness and choking after meals for nearly a year. When he arrived at Max Hospital, Saket he underwent a Barium Swallow Test, which showed that his food pipe (esophagus) was being compressed inside the chest. A more detailed CT aortogram confirmed the presence of an aberrant (abnormally positioned) right subclavian artery – a rare birth defect seen in less than 0.5% of the population. In this condition, instead of following its usual path, the artery grows in a way that it passes behind the food pipe, putting pressure on it and causing difficulty in swallowing, known medically as Dysphagia Lusoria.”


The surgical team decided to treat the condition using robot-assisted surgery, which allowed them to operate with more precision and minimal invasion. First, the abnormal artery in the chest was carefully separated and divided using robotic arms, which relieved the pressure on the food pipe.


Dr. Rajneesh further added “This was a very rare and challenging case. The abnormal artery was not only compressing the food pipe and causing severe difficulty in swallowing, but it also played a vital role in supplying blood to the right arm — which is especially crucial for a growing teenager. So, while we relieved the pressure on the esophagus by dividing the aberrant artery using robotic surgery, we also had to ensure that his right arm continued to receive proper blood flow. To achieve this, we created a new route for blood supply using a special graft to connect the neck and arm vessels. This innovative two-step approach allowed us to solve both issues in a single surgery — relieving the swallowing difficulty and safeguarding the arm's circulation.”


“The robotic-assisted surgery gave us the precision and control we needed for such a delicate procedure. The operation went smoothly, and the boy recovered very well, with complete resolution of his symptoms. He is now eating normally and has returned to his regular life.” Dr Malhotra further added.

Heart disease remains a leading ca use of death in India, with an alarming increase in cases of blocked arteries. Coronary artery disease, which involves the narrowing or blockage of arteries supplying blood to the heart, often necessitates interventions like stenting or bypass surgery. Choosing the appropriate treatment depends on several factors, including the severity and location of blockages, the patient’s overall health, and long-term goals for heart function and quality of life.

In India, over 3 million people are estimated to suffer from coronary artery disease annually. With a rise in cases due to sedentary lifestyles, poor diet, smoking, and diabetes. Understanding treatment options is essential for managing this growing health concern. 


Arteries can become narrowed due to plaque buildup—a mix of cholesterol, fatty deposits, and other substances—reducing blood flow to the heart and increasing the risk of heart attack. This condition can cause symptoms such as chest pain (angina), breathlessness, fatigue, and even heart failure if left untreated. Early diagnosis through tests like ECG, stress tests, and coronary angiography helps determine the extent of blockage and the right intervention.

Stents: Minimally Invasive and Quick Recovery

Stenting, or percutaneous coronary intervention, involves inserting a tiny tube into the narrowed artery to keep it open, improving blood flow. This procedure is less invasive, usually performed under local anesthesia, and patients typically return home within a day or two.

Drug-eluting stents, which release medication to prevent re-narrowing, are commonly used. Stents are ideal for patients with one or two blocked arteries or those who may not tolerate surgery due to age or other health conditions. However, patients must take blood-thinning medications long-term and may face a risk of re-blockage in certain cases. 


Bypass Surgery: Comprehensive Treatment for Severe Blockages

Coronary artery bypass grafting is a more invasive procedure where surgeons use blood vessels from another part of the body to create a new route for blood to flow around blocked arteries. It is typically recommended for patients with multiple blockages, especially in major arteries or in diabetics, where stenting may not provide lasting results.

Though recovery takes longer—hospital stays can last 5–7 days with several weeks of rehabilitation—bypass surgery has proven long-term benefits, including reduced risk of repeat procedures and better outcomes for complex heart disease.

Comparing Outcomes and Making the Decision

Both stents and bypass surgery have their merits. Studies show that while stents are effective for short-term relief and quicker recovery, bypass surgery offers better long-term survival in patients with severe multi-vessel disease. The choice depends on factors like age, co-morbidities (diabetes, high blood pressure), number and location of blockages, and patient preferences. 


It’s crucial to consult a cardiologist or cardiothoracic surgeon to evaluate all options based on individual health conditions. With advancements in minimally invasive bypass techniques and improved stent technology, treatment can be tailored to offer the best outcomes. 


As heart disease cases rise across India, timely intervention is critical. Understanding the benefits and limitations of both stents and bypass surgery can help patients make informed decisions. Early diagnosis, lifestyle changes, and expert consultation are key to preserving heart health and improving quality of life for those with coronary artery disease.
